CDC seeks patient stories: share your experience with DVT/PE

Posted |

The Centers for Disease Control and Prevention (CDC) is planning to launch a Flickr album devoted to personal stories from people who have had a blood clot.  They would like to highlight people who have experienced deep vein thrombosis or pulmonary embolism.  If you would like to participate and briefly share your story, please send an electronic photo and one or two brief paragraphs about your experience in overcoming and managing your disorder.  A consent and release form will need to be signed with CDC.

Click here for the CDC consent and release form

Email the signed release form along with your photo and brief story to:

Cindy Sayers, CDC Health Communications Specialist

Email:  CSayers@cdc.gov

**Please note: A personal signature on the release form is required.  If  you are unable to print, scan and return the signed release form via email, the signed form can be sent by fax or mail at:

Fax:   404-498-6799 ATTN: Cindy Sayers

Mail: Centers for Disease Control, 1600 Clifton Road, MS E 64, Atlanta, GA 30333  ATTN: Cindy Sayers

Clot Connect encourages you to share your personal experience with others. While blood clots are a common medical condition, there exists low public awareness.  Your personal story can be a powerful example to educate others.
